CDC’s PLACES
The Centers for Disease Control and Prevention’s PLACES data portal includes 36 chronic disease measures that can be used to develop targeted prevention activities and identify health problems. This resource adds to existing data by using small area estimation methods to provide estimates for all areas of the U.S. Data is available at the County, Place, Census Tract, and ZIP code tabulation area level. The data can be visualized directly on an interactive web map.
